Release/Waiver
I hereby agree to indemnify and hold harmless KCYA and its employees from and against any and all claims for personal injuries or damages from any kind arising from participating in the CSA program. Further, I authorize KCYA staff and faculty to seek emergency medical help if this becomes necessary. I realize that every effort will be made by KCYA staff to contact me in the event of an emergency involving my child and I agree to indemnify and hold harmless KCYA personnel in seeking medical care for my child.
Refund Policy
Fees will be refunded, less a processing fee, if you withdraw from a class before the deadline. If you must withdraw after the deadline, no refund will be given, except for documented medical reasons. There are no exceptions to this policy. KCYA cannot provide make-up programs, refunds or credits for days missed due to illness, personal schedule conflicts or other reasons.
Photo Consent
I acknowledge and agree that KCYA may document my participation in photos, video and/or audio recordings and may use such documentation for archival purposes and current or future promotions. All such documentation taken by KCYA is owned by KCYA and may be used in any standard publicity practice including social media such as Facebook, Twitter and You Tube. I acknowledge and agree that any such documentation does mean that I may be included in the selected pieces for distribution. I acknowledge and agree that the documentation may be used for publicity in whatever way KCYA deems appropriate and that I will not be notified when and where the documentation is used.
I acknowledge and agree that I will not be compensated for any such photography, video or audio recordings, for any printed images, and I relinquish all rights to any such documentation bearing my likeness.
